Tim Hoynes, PTA/ATC

Trish Strazar, PT

Trish has been a full-time practicing physical therapist for 37 years, and an athletic trainer for 39 years. She enjoys working with athletic injuries, and rehabilitating athletes to return to the sport that they love. She also enjoys working with expectant mothers, and following up on them post-partum. Trish is the mother of 6 athletic daughters, and still enjoys playing soccer, tennis, golf, swimming, skiing, hiking and any other physical activity. Her goal is to get everyone “back in the game of life!” Trish’s energy and enthusiasm are a plus for everyone’s rehabilitation.

Zachary Poreda, PT

Zachary is experienced with adolescent, adult, and geriatric populations, treating a variety of post operative and conservative care diagnoses in the outpatient orthopedic setting.  He focuses on combined manual and exercise based approaches to optimize patient functional performance, attending to the total body influence of dysfunction.

Jourdan Zajaczkowski, PTA

Jourdan joined Southwest General Health Center in August of 2022 after completing her PTA degree through Bryant & Stratton College. She became interested in physical therapy as she attended years of physical therapy sessions with her little brother who has Cerebral Palsy. Her interest in the medical field prompted her to complete the medical program at Normandy while still in high school, and she worked as a home health aide while putting herself through college. Jourdan’s enthusiasm and compassion with her patients helps them to gain mobility and confidence to improve their quality of life.
